Report a Service Issue

Click on the "Report a Service Issue" link on the home page. This link will take you through the steps to create a new Service Issue. If you would like to view or update any existing service issues, you will need to login to the Colorado Courier System.

When you click on the "Report a Service Issue" link, the Courier News section of the page is replaced by a new section. If you know your library's courier code, enter it and click on the "Continue" button. You can also search for the courier code. To Lookup the library courier code, type into the box next to the button all or part of the library name, city or the courier code and click on the "Lookup" button. If there is a single match for your search, the appropriate information will be automatically placed into the field. If multiple matches are found, a list will display below the field, with buttons for you to use to select the appropriate library/courier code. Once the correct code is displayed in the search box, click "Continue" for the next step in the process.

A blank Service Issue form will be displayed for you to complete.

| Your Courier Code | Displays the courier code you entered in the first step of the process. |
| Your Building | Displays the school building selected, if any, in the first step of the process. |
| Issue Type | Select the type of problem you are reporting. Required field. |
| Contact Name | Enter the name of the person that should be contacted regarding this service issue. Required field. |
| Contact Phone | Enter the phone number for the contact person. Required field. |
| Contact Email | Enter the email address for the contact person. Required field. |
| Description | Describe the problem in 255 characters or less. Required field. |
| Notes | Enter any other information you think is important regarding the service issue. |
Once you have filled in as much of the form as necessary, click on the "Open Service Issue" button. A confirmation message will be displayed on the screen, including the unique Service Issue Number associated with the report you just entered.

You can now enter the information for another new ticket or you can click on the "Home" link to return to the Colorado Library Courier Home Page.
